پیش‌بینی پایبندی به درمان برای افراد مبتلا به مولتیپل اسکلروزیس از طریق سبک‌های دلبستگی با واسطه حمایت اجتماعی ادراک‌شده و کیفیت زندگی مرتبط با سلامت

چکیده

مقدمه: مولتیپل اسکلروزیس (MS) یک اختلال عصبی مزمن است که سلامت جسمی و روانی بیماران را به خطر می‌اندازد. پایبندی پایدار به درمان برای مدیریت مؤثر ضروری است، اما اغلب حفظ آن دشوار است. این مطالعه به بررسی تأثیر سبک دلبستگی، کیفیت زندگی مرتبط با سلامت (HRQoL) و حمایت اجتماعی ادراک‌شده بر پایبندی در بین افراد مبتلا به MS می‌پردازد.
روش‌ کار: این مطالعه توصیفی-همبستگی شامل ۲۳۹ بیمار MS از مرکز جامع MS در مشهد-ایران، در سال ۲۰۲۲ بود. شرکت‌کنندگان پرسشنامه‌های معتبر در مورد سبک‌های دلبستگی، حمایت اجتماعی ادراک‌شده، HRQoL و پایبندی به درمان را تکمیل کردند. از مدل‌سازی معادلات ساختاری (SEM) برای تجزیه و تحلیل روابط بین متغیرها استفاده شد.
نتایج: همبستگی‌های دو متغیره نشان داد که دلبستگی اجتنابی و اضطرابی با پایبندی به درمان ارتباط منفی دارند، در حالی که دلبستگی ایمن ارتباط مثبت دارد. با این حال، SEM هیچ اثر مستقیم معناداری از سبک‌های دلبستگی بر پایبندی نشان نداد. حمایت اجتماعی ادراک‌شده (β= 0.353، P= 0.001) و کیفیت زندگی مرتبط با سلامت (β= 0.277، P= 0.003) اثرات مستقیم مثبت و معنی‌داری بر پایبندی به درمان نشان دادند. اثرات غیرمستقیم سبک‌های دلبستگی بر پایبندی از طریق حمایت اجتماعی ادراک‌شده و کیفیت زندگی مرتبط با سلامت (HRQoL) غیرمعنی‌دار بود.
نتیجه‌گیری: یافته‌ها بر نقش کیفیت زندگی مرتبط با سلامت و حمایت اجتماعی ادراک‌شده در ارتقای پایبندی به درمان در بیماران MS تأکید دارند. اگرچه سبک‌های دلبستگی با پایبندی به درمان همبستگی دارند، اما فاقد قدرت پیش‌بینی مستقیم هستند. مداخلاتی که بهبود کیفیت زندگی مرتبط با سلامت و افزایش حمایت اجتماعی را هدف قرار می‌دهند، می‌توانند به طور مؤثر پایبندی را در این جمعیت افزایش دهند.
