مقایسه اثربخشی درمان روان‌پویشی فشرده و درمان شناختی رفتاری متمرکز بر هیجان مبتنی بر مدل SPARS بر نشخوار فکری و افسردگی در زنان نابارور

چکیده

مقدمه: افسردگی و نشخوار فکری در بین زنان نابارور شایع است و نیاز به مداخلات مؤثر دارد. این مطالعه اثربخشی درمان روان‌پویشی فشرده (IPT) و درمان شناختی رفتاری متمرکز بر هیجان مبتنی بر SPARS (EF-CBT) را بر نشخوار فکری و افسردگی مقایسه کرد.
روش‌ کار: در این مطالعه، 60 زن نابارور در تهران-ایران در سال 2024، به صورت هدفمند نمونه‌گیری و به طور تصادفی به دو گروه مداخله و یک گروه کنترل (هر کدام 20 نفر) اختصاص داده شدند. از پرسشنامه نشخوار فکری نولن-هوکسما و پرسشنامه افسردگی بک استفاده شد. IPT شامل 8 جلسه گروهی (2 جلسه در هفته، هر جلسه 60 دقیقه) بود، در حالی که EF-CBT مبتنی بر مدل SPARS شامل 12 جلسه (2 جلسه در هفته، هر جلسه 60 دقیقه) بود. گروه کنترل در لیست انتظار قرار گرفت. داده‌ها با استفاده از آزمون‌های ANCOVA، ANOVA و t-test تجزیه و تحلیل شدند.
نتایج: تفاوت‌های معناداری بین گروه‌های مداخله و کنترل در نمرات پس‌آزمون نشخوار فکری (P= 0.000، F= 175.118) و افسردگی (P= 0.000، F= 61.57) مشاهده شد. EF-CBT تأثیر بیشتری بر نشخوار فکری و افسردگی نسبت به IPT داشت.
نتیجه‌گیری: هر دو EF-CBT و IPT مبتنی بر SPARS به طور مؤثری نشخوار فکری و افسردگی را در زنان نابارور کاهش می‌دهند. آگاهی و پذیرش احساسات اساسی، بازسازی شناختی و ابراز احساسات، مکانیسم‌های کلیدی هستند. این مداخلات را می‌توان در کلینیک‌های ناباروری و مراکز مشاوره به کار برد.
