Some studies have been conducted about the role of gabapentin in the treatment of bipolar disorder in adults. This study was designed to investigate the efficacy of combined gabapentin and lithium treatment in youths with bipolar disorder.  Materials and Methods: This is a randomized placebo-controlled clinical trial in 2009-2010. In an 8-week, 31 adolescents with diagnosis of bipolar disorder were randomized for treatment with lithium plus gabapentin (n=16) or lithium plus placebo (n=15). The dosages of lithium and gabapentin gradually increased to therapeutic serum level 08-1.1 meq per liter and 400 mg, respectively. The primary outcome measure was Young mania rating scale score (YMRS). Anxiety and depression were measured biweekly with Hamilton rating scale (HARS) and Beck depression inventory (BDI). The serum level of lithium and side effects evaluated weekly and biweekly respectively. Also patients were weighed at the beginning and the end of the study. The measures were analyzed using analysis of variance and student t-test.  Results: There were no significant differences between the two groups in YMRS, HARS and BDI scores at baseline. The differences in scores at any stage during the 8-week trial were significantly in both groups (P<0.05) but there were not significant differences between the two groups.  Conclusion: Gabapentin as combination agent with lithium had not significant effects compared to placebo in reducing the symptoms of mania, depression and anxiety.
