In different parts of Iran, between 1.39- 9.50% of patients that attempted suicide and 25.0-40.3% of patients who committed suicide have been reported through deliberate self-burning. Self-burning (immolation) comprises between 0.37% and 40% of total burn center admissions around the world and in Iran, it is comprises between 4.1% and 36.6% of admissions in Iranian burn centers. Approximately, 80% of hospitalized self-immolation patients die. The goal of this study was to identify the epidemiologic features, causes and potential protective factors of suicide by burning in Kermanshah province in the west of Iran, and to develop the effective intervention programs based on the Public Health Approach to Prevention. Methods and Materials: Over the course of a year, from 21 March 2004 up to 20 March 2005 (Based on Iranian Calendar), the authors examined all the suicidal patients who were admitted to the emergency department of hospitals in Kermanshah University of Medical Science. Examination included a retrospective cross-section observational study, via demographic questionnaires and suicidal checklists. Results: Over the period of study, 1820 patients who attempted suicide and 90 patients who successfully committed suicide were admitted. 41% (37 cases) of patients who committed suicide were via self-immolation. 81% of self-immolation patients were female (p<0.0005), and the female: male ratio was 4.3. 86.6 and a half percent of the patients were in the 11- 30 years of age group. The mean age was 24.9 years (range: 14 - 50 years). Forty-nine percent of cases were single, 84% were illiterate or in low level of education. 78.5% were housewives, and 14% were unemployed. Also, most of the patients (53%) were living in rural area, ninety-seven percent did not have a previous history of suicide attempt, and 67.5% regretted their self-immolation act. The majority of the self-immolation (64.8%) occurred during daylight hours, and the most common self-immolation motivation factor was marital conflict (32.5%). Conclusion: Overall, this study demonstrates that self-immolation should be considered as a mental health problem in our society. It is necessary to implement programs and strategies to prevent this problem. The public health approach provides a framework for a National Prevention Strategy to address this serious national problem. Local data on victims and victim stories from self-immolation provided the stimulus for community action}, keywords = {Suicide,Self-immolation,Prevention Strategies,Kermanshah,Iran}, title_fa = {مطالعه اپیدمیولوژی موارد خودسوزی موفق پذیرش شده در بیمارستان های دانشگاه علوم پزشکی کرمانشاه (سال 1383)}, abstract_fa = {خودکشی با آتش در کشورهای غربی بسیار نادر است (06/0 تا 1% تمام خودکشی ها)، اما در کشورهای در حال توسعه نسبتاً شایع است (تا حد 3/ 40% تمام خودکشی ها). در نقاط مختلف ایران حدود39 / 1تا 5/9% از مواردی که اقدام به خودکشی ( ناموفق ) می کنند و 25 تا 3/40% مواردی که خودکشی موفق می کنند، به علت خودسوزی بوده است. آمار رسمی در سال1380 نشان دهنده این موضوع است که 4/36% کل خودکشی های موفق در ایران به علت خودسوزی بوده است. خودسوزی از 37/0% تا 40% از بیماران بستری شده در بخش سوختگی را در نقاط مختلف جهان به خود اختصاص داده است، و در ایران 1/4% تا 6/36% از بیماران بستری شده در بخش های سوختگی نقاط مختلف ایران، به علت خودسوزی بوده است. این در حالی است که 80% از بیماران خودسوزی بستری شده به علت شدت بالای سوختگی می میرند. هدف از این مطالعه شناخت چهره اپیدمیولوژیک خودسوزی موفق در استان کرمانشاه است. نتایج حاصل از این بررسی به عنوان پایه ای جهت مطالعات مداخله ای پیشگیری کننده از این اقدام خود مخرب انسانی به کار گرفته خواهد شد. روش کار: در طول سال 1383 تمام موارد خودکشی پذیرش شده در بیمارستان های دانشگاه علوم پزشکی مورد بررسی قرارگرفتند. نوع مطالعه گذشته نگر مقطعی با استفاده از پرسش نامه جمعیت شناسی و پرسش نامه خودکشی بوده است. نتایج: در مدت یکسال زمان مطالعه، 1820 مورد اقدام به خودکشی (ناموفق) و 90 مورد خودکشی موفق پذیرش شدند. 41% (37 مورد) از خودکشی های موفق به علت خودسوزی بوده است. 81 % از بیماران خود سوزی مونث (0005/0p<)، و نسبت زن به مرد برابر 3/4 بود. 86 % در گروه سنی 11 تا 30 سال بودند. میانگین سنی 9/24 سال بود(دامنه 14 تا 50 سال). 49 % مجرد بودند. 84% سواد کمتر از مقطع راهنمایی داشتند. 5/78% خانه دار و 14% بیکار بودند. همچنین 53% در روستا زندگی می کردند، 97% فاقد سابقه اقدام به خودکشی بودند، 5/67% در موقع ورود به بیمارستان از کار خود پشیمان بودند. زمان غالب جهت انجام خود سوزی در طول روز (8/64%) و شایع ترین عامل محرک جهت خود سوزی، دعوای مابین زن و شوهر بود (5/32%). نتیجه گیری: در مجموع این مطالعه نشان می دهد که خودسوزی باید به عنوان یک مسئله مهم بهداشت روانی در جامعه ما مورد توجه مسئولان قرار گیرد. استراتژی ها و برنامه های پیشگیری باید در اولویت قرار گیرند. سیستم ارائه خدمات بهداشتی درمانی یک بستر مناسب جهت اجرای برنامه ملی پیشگیری از خودسوزی می باشد. آگاهی از آمار قربانیان خودسوزی و سرگذشت آنها انگیزه کافی را جهت تلاش برای پیشگیری از این بلای خود مخرب که دارای عوارض شدید اجتماعی نیز هست، فراهم می کند.}, keywords_fa = {خودکشی,خودسوزی,پیشگیری,کرمانشاه}, url = {https://jfmh.mums.ac.ir/article_1816.html}, eprint = {https://jfmh.mums.ac.ir/article_1816_27e2989558650a598bb6853ade29083b.pdf} }
